We welcome referrals and are committed to making the process simple, efficient, and responsive for both patients and referring providers.

At Stony Creek Dermatology, patients are seen in a timely manner, and we prioritize clear communication back to your office after evaluation.

Most referrals can be scheduled within 1–2 weeks, and we offer same day appointments for urgent matters.

Please complete and save this form, then email it to referrals@stonycreekdermatology.com or fax it to 586-786-8851. For urgent referrals, please also call our office at 586-786-8850.

To help us coordinate care efficiently, please include:

• Patient name and contact information

• Date of birth

• Insurance information (if available)

• Reason for referral

• Relevant clinical notes, labs, or pathology reports (if applicable)
